Abstract: At 11:30 a.m. on April 6, 2021, a coworker loaded welded steel pipe onto a Toyota Forklift. The coworker used the forks to hoist the bulky and unsecured load until the pipe was an inch or more off the ground in order to weld angle iron onto the pipe ends. He had asked an employee to stand by and be available in the event that he needed something (materials, tools, equipment). The pipe then fell, striking the employee in the head and killing him.
