Abstract: At 4:15 p.m. on August 4, 2020, an employee was working for a manufacturer of wood products. He was operating an 1,800-kilogram (2-ton) CM Lodestar overhead hoist to lift a dock section. The dock section was 13.8 meters long by 1.2 meters wide (45 ft long by 4 ft wide). Two coworkers were building the dock. They had temporarily rigged it with a crowbar to lift it up only a few millimeters or inches. They were eventually going to place a proper sling around it. After placing the temporary rigging, they went to get carts to place the dock on. The employee went over to the controls and lifted the section a few feet, unaware that the temporary crowbar rigging was still in place. When he recognized that the temporary rigging was still in place, he let the dock down. The temporary rigging slackened, and the dock section fell. It came down on the employee's legs and ankles. The employee suffered fractures to his legs and ankles. While he was hospitalized, he suffered a deep vein thrombosis (DVT) clot and died.
