Abstract: At 1:00 p.m. on February 23, 2019, Employee #1 was in a confined space as a 1,20 0 pound steel bar screen that was being lifted by an overhead crane. The sling h olding the bar screen failed, causing it to fall and strike the employee. The em ployee suffered a concussion, broken ribs, and back injuries. He was hospitaliz ed for several days.
