Investigation Summary
At 12:19 p.m. on August 14, 2020, an employee was a part of a maintenance team working to tighten a nut on a granulator. The employee and the coworkers were using a spanner wrench with cheater bar that was approximately 106 inches long to increase leverage. As the employee and the coworkers were torqueing the nut with the spanner wrench, the lug on the spanner wrench sheared off. The employee was standing on a platform and fell backward. The employee struck his head on the floor and was killed.
