Investigation Summary
At 1:17 p.m. on April 19, 2018, an employee was feeding book pages into the Wohlenberg Quickbinder machine that installs the outer covers on the pages. The employee inserted book pages into the conveyor by dropping them down but kept his hand on them as it entered the area that has a clamp. The employee caught his finger under the clamp and amputated the tip of it when pulling it from the machine. The employee was hospitalized.
