29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about October 1, 2014, employees were exposed to electrical and struck-by hazards when the employer had not developed a lockout/tagout program or machine specific procedures to document how to lockout/tagout the equipment. The employer has lathes, mills, saws, broaches, CNC machines, and other metal working machines that require maintenance. Equipment such as, but not limited to, band saws and broaches require the use of lockout/tagout during machine setup or tool change.
