29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146: Employees were exposed to injury from the hazards associated with confined space entry. A written confined space entry program that addressed the hazards of entering furnaces and rotary furnaces, which meet the definition of permit required confined spaces, had not been developed or implemented. There were 3 furnaces and 3 rotary furnaces in the production area that were periodically entered by employees. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is complete.
