Text For Citation: 01 Item/Group: 006A Hazard:
29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. At the site, equipment specific lockout/tagout procedures were not developed and implemented to include at a minimum the following: 1. A specific statement of the intended use of the procedure. 2. Specific procedural steps for shutting down, isolating, blocking, and securing machines or equipment to control hazardous energy. 3. Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them. 4. Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices and other energy control measures. 5. The magnitude of hazardous energy. Employees were required to conduct maintenance on equipment and machines such as, but not limited to, two compressors, an alligator shear, two down stroke bailers, a REB two ram Baler, a T 900 ton Shear, two forklifts, a Bobcat S-300, and a Sennebogen.
