29 CFR 1910.147(c)(8): Lockout or tagout was not performed only by the authorized employees who are performing the servicing or maintenance: The employer does not ensure that only the authorized employees who are performing service and maintenance work are locking or tagging out machines and equipment. This violation occurred on or about August 5, 2014, and at times prior thereto, at the facility where employees were exposed to struck-by and electrical hazards while performing maintenance and servicing work on an airport shuttle train system without having the employees performing the servicing work lock or tag out the energy sources. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the employees that are authorized to perform service or maintenance work on machines and equipment are using a lockout or tagout system that is under their control.
