29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: The employer is failing to protect workers from fall hazards when performing steel erection activities. This violation was most recently documented at a worksite located at 3500 East 149th Street, Kansas City, Missouri, where fall protection devices such as harnesses, shock-absorbing lanyards, retractable lanyards, and anchor devices were not inspected prior to use. To abate this violation, the employer must assure that supervisors and employees are trained to and inspect personal fall arrest systems prior to each use for wear, damage, deterioration, and/or defective components. Pursuant to 29 CFR 1903.19, within ten (10) days of the abatement date, the employer must submit documents showing that it is in compliance with the standard, including the inspection of personal fall arrest systems prior to each use for wear, damage, deterioration, and/or defective components by a supervisor or competent person with the authority to remove and replace worn, damaged, deteriorated and/or defective equipment.
