29 CFR 1910.119(d)(3)(ii): The employer shall document that equipment complies with recognized and generally accepted good engineering practices. a) Mobile Bay Gas Processing Plant - On June 25, 2014 and at times prior employees were exposed to fire and explosion hazards due to a car seal missing on the four inch gate valve 040019 which is down stream from PSV 4001 on drawing MOB-8-P-040. Drawing MOB-8-P-040 did not show a car seal open on gate valve 040019 which is up stream from PSV 4001. The employer did not comply with the provisions of API 520 Part II Paragraph 6.5 Sizing, Selection and Installation of Pressure -Relieving Devices which requires that administrative control be in place that will prohibit the inappropriate closing of isolation valves in pressure relief system piping b) Mobile Bay Gas Processing Plant - On June 25, 2014 and at times prior employees were exposed to fire and explosion hazards due to drawing MOB-P-039 missing a car seal open on the 1 1/2 inch gate valve 039019 which is up stream from pressure control valve 3905. The employer did not comply with the provisions of API 520 Part II Paragraph 6.5 Sizing, Selection and Installation of Pressure -Relieving Devices which requires that administrative control be in place that will prohibit the inappropriate closing of isolation valves in pressure relief system piping .
