Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards associated with pressurized equipment. On or about April 30, 2014 employees engaged in flowback operations at 4H well head were exposed to struck-by hazards due to the failure of a pressurized system. Feasible and acceptable methods of correcting the hazard, among others is to: 1) Ensure that vessels subjected to wellhead pressures are designed, manufactured and utilized in accordance with ASME, Section VIII, Division1, UG-116 and 125 and the National Boiler and pressure Vessel Code. 2. Ensure that pressure vessels are equipped with properly sized pressure relief devices as outlined in API 12J, Section A.2.6. 3. Ensure pressurized piping is secured using restraint devices that will resist pipe whipping should piping become disconnected or broken as outlined in API 74. 4. Implement a program of inspection, testing and removal from service for pressure vessels and piping as outlined in API.
