29 CFR 1910.272(g)(1)(ii): Prior to entry, all mechanical, electrical, hydraulic, and pneumatic equipment which presented a danger to employees inside grain storage structures in the grain handling facility were not disconnected, locked out and tagged, blocked-off, or otherwise preventing operation by other equally effective means: (a) On March 14, 2014, the Employer permitted an employee to work inside a grain bin with an energized sweep auger to operate the sweep auger from a position behind the sweep auger, but the Employer did not implement engineering and administrative controls to ensure the employee operating the sweep auger remained outside the danger zone of the energized sweep auger at the railcar load out elevator, 500 Fifth Street NE, Lake Preston, South Dakota. (b) On or about March 14, 2014, for employees exposed to caught-in and/or amputation hazards where grain storage bins were entered while the bin conveying auger was operated without a cover to prevent an employee from stepping into the opening above the auger and potentially contacting the moving parts of the auger at the railcar load out elevator, 500 Fifth Street NE, Lake Preston, South Dakota.
