29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: Jobsite in garage: On or about August 16, 2013 employees were exposed to electrical shock hazards while plugging and unplugging an extension cord where the outer protective casing had pulled away from the plug exposing the live conductor wires that was not GFCI protected.
