Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a) Clarion Boards, Inc. / 143 Fiberboard Road, Shippenville, PA: On or about August 23, 2013, Employees were potentially exposed to struck-by hazards from potentially defective hoists due to the lack of a periodic inspection program. The thirty nine hoists in the facility had not undergone a documented periodic inspection for a time frame greater than one year. The hoists included but were not limited to Model BEH0208 2-ton, 1-ton Kone Crane #033, 5-ton Kone Crane #035, and the JLP-75-20 ¾-ton. Hoist deficiencies included but were not limited to twisted load chains, broken chain bucket mounts, missing capacities, damaged cords, damaged hooks, and damaged hooks.
