29 CFR 1910.147( C )(7)(i): The employer had not ensured that the purpose and function of the employers energy control program were understood by employees. (a) Alpine Waste and Recycling, 645 W. 53rd Place, Denver, CO: Prior to authorizing an employee to operate a company owned Heil 5000 rear loading trash collection truck, the employer had not provided adequate training to ensure the purpose and function of the employers energy control program were understood. On or about May 3, 2013, an employee of the company was fatality injured after the hydraulic packer panels activated while the operator was inside the rear hopper section of the compactor. Alpine Waste and Recycling was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(C)(7)(i), which was contained in OSHA inspection number 313415119, citation number 1, item number 2b and was affirmed as a final order on March 9, 2010, with respect to a workplace located at 645 W. 53rd Place, Denver, CO. Abatement note: Per 29 CFR 1910.147( C )(7)(iv), the employer shall certify that employee training has been accomplished and is being kept up to date. The certification shall contain each employees name and dates of training.
