29 CFR 1910.119(f)(1)(i)(C): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting temporary operations. On or about June 14, 2012, and at times prior thereto, the employer did not establish written procedures or clear procedures, such as but not limited to: a) Unloading of Anhydrous Ammonia b) In the Anhydrous Ammonia unloading building, the employers supervisor overrides the ammonia monitoring system as a matter of practice, disabling the ammonia alarms, each time during the unloading process.
