29 CFR 1910.305(g)(2)(ii): Flexible cords were not used in continuous lengths without splice or tap: a) Magpie Operating, Inc. at LBDSU at CR 24 and CR 8, Last Chance, CO 80757: On or before May 10, 2012, the employer allowed the use of two extension cords spliced together wtih black tape. The black tape did not maintain the insulation, outer sheath properties, and usage characteristics of the cord being spliced. The cord was used to power a portable utility heater, model LH-872, sn 3105544. The longer, spliced extension cord had the potential to overheat at the heater's 1500 watt setting, and to ignite the hydrocarbon and water mixture on the floor. Condition exposed employees to the electrical shock and burn hazards.
