29 CFR 1910.119(e)(3)(iii): The process hazard analysis (PHA) did not address engineering and administrative controls applicable to the hazards and their interrelationships with the covered process: a) 275 Steigerwalt Hollow Road facility - The employer did not adequately evaluate and identify the engineering and administrative controls related to temperature hazards in the PHA for the Chlorine Feed System, on or about May 8, 2012. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
