Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment wh ich were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by/crushing hazards: THIS CITATION IS ISSUED TO THE CORRECTING, CREATING AND EXP OSING EMPLOYER UNDER OSHA'S MULTI-EMPLOYER WORK-SITE POLICY. a) Shoring Towers # 5 through #11 had heights exceeding four times the minimum base width and were n ot guyed or braced, exposing employees to a fall of over 30' and/or being struck by or crushed by the collapse of the Atlanta Botanical Gardens (ABG) Canopy Wal kway. Among other methods, one feasible and acceptable abatement method to corre ct this hazard is to follow the Safe Practices for Erecting and Dismantling of F rame Shoring, per the Scaffolding, Shoring, and Forming Institute, Inc. (SSFI), Section II.G., which states, "Use caution when erecting free standing towers. Prevent tipping by guying or bracing when height exceeds 4 times the minimum base dimension."
