29 CFR 1910.146(d)(11): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement procedures to coordinate entry operations when employees of more than one employer were working simultaneously as authorized entrants in a permit space, so that employees of one employer did not endanger the employees of any other employer: Employees entered tank 242 under a confined space permit to perform maintenance and cleaning. During the cleaning processes levels of flammable vapor in the tank changed; this increased hazards to the maintenance contractor employees.
