Text For Citation: 01 Item/Group: 001 Hazard: BURNS
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees working in the snubbing unit basket who were exposed to burns from a fire and blow out at the well bore. a) On or about June 8, 2004, the employer, Cudd Pressure Control Inc, did not ensure that emergency escape lines were rigged and available to each employee in the snubbing unit basket at the Leland Shroeder Well # 1 in Franklin Tx. The snubbing unit basket was rigged with 2 emergency escape lines that burned when on or about June 8, 2004, a blow out and fire occurred at the well site. Among other methods, recognized and feasible means to correct this hazard include, but are not limited to the employer following the guidelines recommended by the American Petroleum Institute Recommended Practice 54 14.2.2 which states that individual emergency escape lines should be rigged and made available for each person working atop hydraulic snubbing equipment.
