Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the A-Frame and/or components of the A-Frame system on the Kenworth C-500 truck: a)D & D Water Service & 3 miles N.E. of Strasburg Road and 88th Ave., Strasburg, CO: On or before May 13, 2003, the employer did not ensure that the mast (A-Frame) system on the Kenworth C-500 truck was substantially constructed to conform to good engineering practice and maintained in safe condition: 1)The method chosen by the company to raise the mast caused an uneven and improper load to the A-Frame dogbone. 2)The A-Frame hoist was improperly modified. 3)The A-Frame mast system did not include a secondary hold-back system which was independent of the primary winch/cable holdback system. Such a secondary hold back (approved by an engineer familiar with the requirements of such equipment) would restrain the movement of the mast in the event of a primary holdback system failure. Among other methods, one feasible and acceptable method to correct the hazard is to conform with the requirements of American Petroleum Institute Recommended Practice 54 1999, Section 9, Sub-Section 9.2, Paragraph 9.2.1. The company must determine a method to raise the mast which does not place uneven and improper loading to the A-Frame dogbone. Abatement note: Abatement certification and documentation are required from this item (see enclosed "Sample Abatement Certification Letter"). b)D & D Water Service & 3 miles N.E. of Strasburg Road and 88th Ave., Strasburg, CO: On or before May 13, 2003, the employer did not ensure that the A-Frame system on the Kenworth C-500 truck had a permanent name plate attached to the structure indicating the name of manufacturer, model number and serial number, rating including static hook load capacity with number of lines, and whether guying is applicable and the recommended guying pattern: Among other methods, one feasible and acceptable method to correct the hazard is to conform with the requirements of American Petroleum Institute Recommended Practice 54 1999, Section 9, Sub-Section 9.2, Paragraph 9.2.2. Abatement note: Abatement certification and documentation are required from this item (see enclosed "Sample Abatement Certification Letter"). c)D & D Water Service & 3 miles N.E. of Strasburg Road and 88th Ave., Strasburg, CO: On or before May 13, 2003, the employer did not ensure that the rigging on the Kenworth C-500 truck A-Frame system was adequate in that the working line on the #1 winch terminated in a knot, weakening the wire rope and exposing employees to a falling load. Among other methods, one feasible and acceptable method to correct the hazard is to conform with the requirements of American Petroleum Institute Recommended Practice 9B 2002, Section 3, Sub-Section 3.7, Paragraph 3.7.6.
