Section 50-9-5(A), NMSA 1978 (New Mexico Occupational Health and Safety Act): Every employer shall furnish to each of his employees employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm to his employees. At the Pure Resources Well Site, Rincon Unit No. 166R, the Gardner-Denver Tee Triplex Well Servicing Pump located on the 2001 Mack RD6 truck (VIN#1M2P267C21M056927), did not have a safety relief valve when a AOP Valve was placed on the discharge side on the pump. The Denver Tee Triplex well servicing pump exceed normal operating pressures contrary to API RP 54 - 8.13.3 which states: In normal operations, pumps, piping hoses, valves, and other fittings shall not be operated at pressures greater than their rated working pressure and shall be maintained in good operating condition. Date Corrected: 5-15-02 Describe Corrective Action: Abatement Completed. Employer removed AOP Valve on the discharge of the pump.
