29 CFR 1910.120(q)(2)(ii): The emergency response plan did not address, to the extent not addressed elsewhere, personnel roles, lines of authority, and communication. On or about July 16, 2021, for the tank farm, the employer did not update the emergency response plan to identify personnel roles to handle emergencies involving flammable liquids. This exposed employees and contract employees to fire and explosion hazards.
