29 CFR 1910.119(j)(6): Quality assurance. 29 CFR 1910.119(j)(6)(ii):Appropriate checks and inspections were not performed to assure that equipment was installed properly and consistent with design specifications and the manufacturer's instructions. a. On or about and prior to April 8, 2021, the employer failed to assure that appropriate checks and inspections were performed to assure that equipment, such as, but not limited to, the kettle 3 pressure vessel, rupture disks and gaskets, were installed properly and consistent with design specifications and manufacturer's instructions exposing employees to fire and explosion hazards from loss of containment events related to the alteration and repair of equipment. On April 8, 2021, the kettle 3 reactor vessel manway failed and released vapor into the kettle 3/kettle 4 room. This catastrophic failure resulted in the explosion event that resulted in a fatality and multiple disabling injuries .
