Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) Dry Ice Production Facility, Delaware City, DE - On or about October 9, 2020, the employer failed to protect employees from amputations and other hand injuries by not: 1. apprising an authorized employee of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy before a machine such as, but not limited to, an IceTech SL1000 dry ice maker was shut down; 2. ensuring that an orderly shutdown of a machine such as, but not limited to an IceTech SL1000 dry ice maker, was utilized to avoid any additional or increased hazards to employees as a result of equipment stoppage; and, 3. installing components an electronic interlock system in a physical location to allow for the de-energization of parts, such as, but not limited to, the rotating part within the ice box in a IceTech SL1000 dry ice maker, in order to prevent employee injuries, such as hand lacerations and amputations.
