29 CFR 1926.1204(c)(6): The entry employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including, but not limited to: Providing pedestrian, vehicle, or other barriers as necessary to protect entrants from external hazards: a) On June 29, 2020, at the above address, the employer did not protect a 6th floor hoist-way opening of approximately 12.5" from external hazards posed by materials or equipment located in the hallway such as but not limited to, a horizontal spindle used for a reel stand system, thereby exposing an employee working in the elevator's pit to struck-by hazards. One reel of hoist cable was suspended on reel stands (two Greenlee Model 687 supports and a horizontal spindle of unknown origin). This equipment was located in the 6th floor hallway just outside of a hoist way opening of approximately 12.5" (between the bottom of the elevator car and the floor of the hallway), which was not adequately protected by a fixed toe board or other guard. The horizontal spindle was not approved by the manufacturer for that use and was therefore subject to falling from the reel stand (as recognized by the manufacturer).
