29 CFR 1910.147(c)(7)(i)(A):Each authorized employee did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) The employer did not ensure that employees performing cleaning, and replacing tools such as but not be limited to drill bits, blades and spindle heads on KOMO machines were trained in the purpose and use of the energy control procedure. Employees were exposed to struck-by, crushed-by moving parts, and/or amputation hazards.
