29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee. 230 Castlewood Drive, Berlin, CT: On or about June 8, 2020, a Werner rooftop anchor was used as part of a fall protection system. Seven nails were used to secure the anchor to the rooftop, when a minimum of 12 nails were required. The nails that were used were not used in accordance with the manufacturers installation requirements. This exposed employees to falls of heights between 20 and 30 feet.
