OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees a) Seale, AL; On or about June 1, 2020, the employer allowed an employee to operate a capstan hoist during the decommissioning of equipment on a 349' guyed cell phone tower without ensuring that the employee was qualified as a competent rigger or without having someone qualified as a competent rigger to supervise his actions exposing employees to crush by hazards. b) Seale, AL; On or about June 1, 2020, the employer allowed two employees to work on a 349' guyed cell phone tower during the decommissioning of equipment without ensuring that the employees were supervised by a competent climber exposing the employees to fall hazards. c) Seale, AL On or about June 1, 2020, the employer created and implemented a Class II rigging plan that was inadequate in that it did not provide for the safety of personnel and for the stability of the structure and lifted components in that possible complications were not addressed or how, who, or with what equipment they are addressed.
