29 CFR 1910.178(a)(4): Modifications and additions which affected capacity and safe operation were performed by the customer or user without manufacturers written approval, and the capacity, operation and maintenance instruction plates, tags or decals were not changed accordingly: a. 900 Fairmount Ave, Elizabeth, NJ - Warehouse The employer authorized the use of attachments to powered industrial trucks without the written approval from the manufacturer. Employees unloading 55 gallon drums of frozen food products used an Easy Lift Equipment, Eagle-Grip Attachment on the forks of a Crown, 5200 Series, forklift for loading and unloading trailers without an identification plate that included capacity of the forklift with the attachment.
