Text For Citation: 01 Item/Group: 002B Hazard:
29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. The employer is failing to protect employees from fall hazards. This was most recently documented on or about 11/30/19 at the establishment located at 510 E. Trail St. in Dodge City, Kansas. The employees were exposed to fall hazards of approximately 15 feet 6 inches to the concrete and piping below in that during the removal of ceiling insulation and panels, employees were anchored to a 7/8 inch cross brace not certified to support at least 5,000 pounds per employee attached.
