29 CFR 1926.416(a)(3): Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit. The employer also did not post and maintain proper warning signs where such circuits exist, or advised employees of the location of such lines, the hazards involved, and the protective measures to be taken: a) 2812 W. Master St., Philadelphia, PA 19121 - The employer did not ascertain whether an energized primary electrical circuit conductor of 7,600 volts, approximately within one foot of the top walking/working surface of the scaffold, was so located that the performance of work by an employee on a fabricated frame scaffold, approximately 34.5 feet high, 28 feet long and 5 feet wide, could bring an employee, tool, or materials into physical or electrical contact with the circuit; did not post warning signs; and did not advise the employee of the location, hazards, and protective measures of the circuit, thus exposing the employee to electric shock hazards. This condition was noted on or about November 20, 2019.
