Text For Citation: 01 Item/Group: 001 Hazard:

OSH ACT of 1970 SECTION (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing, pinning, and struck-by hazards: The employer is failing to protect employees from amputation, crushing, pinning, and struck-by hazards. This was most recently documented on or about November 20, 2019 at CNH Industrial, LLC. located at 3301 S. Hoover Road Wichita, Kansas. The employer exposed employees to amputation, crushing, pinning, and struck-by hazards while operating the Funac Robotic Arm, Model: M-2000iA 900L in the 260 OP welding workstation, which was improperly safeguarded. Among other methods, one feasible and acceptable means to correct this hazard includes but is not limited to the following: 1) Comply with the FANUC Robot M-2000iA MECHANICAL UNIT OPERATOR'S MANUAL MAROCM20003091E REV. G supplied by the manufacturer to perform a risk assessment to determine what safeguarding is best for the process to protect the employees. 2) Install safeguarding devices in accordance with Clause 10 of ANSI/RIA R16.06-1999 (Robots and Robot Systems - Safety Requirement). 3) Install a local alarm fault reset that can only be initiated at that workstation by the operator who knows what is going on at that time in accordance with ANSI B11.19 - 2010 American National Standard for Machines - Performance Criteria for Safeguarding, Section 6.5.4 "Reset of the perimeter safeguarding shall only be performed after the hazard zone is clear of all individuals".
