OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crush/struck by hazards: a) In the elevator hoistway door facing out to the sidewalk; the door interlock was bypassed, allowing the elevator to operate with the door open; employees were exposed to crush/struck by hazards as the elevator moved up to the second floor. b) On the hoistway door facing into the woodshop at the 2fl; the door was modified with a pull chain to allow the elevator door to be opened from the hallway whether or not the elevator car was at the floor landing; employees were exposed to crush/struck by hazards by the elevator car moving by the 2nd floor landing. Location: 304 Boerum Street, Brooklyn NY On or about 8/2/2019 Follow ANSI A17.3-2002 Safety Code for Existing Elevators, Section 2.7.1b Hostway Door or Gate Locking Devices Interlocks Required for Freight Elevators; Hoistway doors or gates for freight elevators shall be equipped with hoistway-door interlocks except as covered by (c) NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.
