29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer is failing to protect employees from amputation and struck-by hazards while performing pumping unit maintenance activities. This was most recently documented on or about July 3, 2019 at the Hatcher D-1 Well site of Merit Energy Company, located at Road 11 and Road L in Liberal, Kansas. Employees engaged in pumping unit maintenance activities at the Lufkin pumping unit were exposed to amputation and struck-by hazards associated with the rotating/moving pumping unit components in that machine guarding was not provided. a) The employer failed to provide guards to prevent employees from getting into the area of the moving parts of the pumping unit such as the horsehead, and the carrier bar exposing employees to struck-by hazards. b) The employer failed to ensure machine guards provided were adequately maintained for the crank, the prime mover, and the counter-weight exposing employees to amputation hazards. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is complete.
