29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about June 7, 2019 the employer failed to ensure that effective machine guarding was in place on line 102-2. A guard was not present to protect employees from the shaft driven rollers before the blade apparatus. c) On or about June 7, 2019 the employer failed to ensure that effective machine guarding was in place on line 102-3. A guard was not present to protect employees from the shaft driven rollers before blade apparatus. d) On or about June 7, 2019 the employer failed to ensure that effective machine guarding was in place on line 102-3. A broken guard was present that did not protect employees at the main control areas. e) On or about June 7, 2019 the employer failed to ensure that effective machine guarding was in place on line 102-3. A guard was not present to protect employees from the moving machine parts at the final exit/ stacking area. f) On or about June 7, 2019 the employer failed to ensure that effective machine guarding was in place on line 102-3. Guards were not present to protect employees from the moving machine parts at the pump units. h) On or about June 7, 2019 the employer failed to ensure that effective machine guarding was in place on line 102-13. An interlock mechanism was not in place on a barrier guard to protect employees from the moving machine parts of the equipment. i) On or about June 7, 2019 the employer failed to ensure that effective machine guarding was in place on line 102-14. Effective guarding was not in place in multiple areas of the line including but not limited to the printing area, folding area, and drying area. Some barrier guarding was in place but did not prohibit employees from reaching into the moving machine parts of the equipment. j) On or about June 7, 2019 the employer failed to ensure that effective machine guarding was in place on line 102-15. Effective guarding was not in place in multiple areas of the line including but not limited to the printing area, folding area, and final exit area. Some barrier guarding was in place but did not prohibit employees from reaching into the moving machine parts of the equipment. k) On or about June 7, 2019 the employer failed to ensure that effective machine guarding was in place on line 202-01. A broken non functioning interlock was equipped on a hinged guard to protect employees from the moving machine components of the printing area. l) On or about June 7, 2019 the employer failed to ensure that effective machine guarding was in place on line 202-8. A guard was not present to protect employees from the powered rollers of the floor unit. Envelope 1 was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1074333, citation number 1, item number 1 and was affirmed as a final order on 12/22/16, with respect to a workplace located at 41969 State Route 344, Columbiana, OH 44408.
