29 CFR 1926.754(e)(3)(i): Covers for roof and floor openings shall be capable of supporting, without failure, twice the weight of the employees, equipment and materials that may be imposed on the cover at any one time. (a) On or about the 3-day period of April 29 - May 1, 2019, located at the south end of a commercial metal roof structure, two employees tied off to a perimeter cable system were exposed to fall hazards while removing screws from plastic skylight panels in that 10 uncovered plastic skylight panels were not capable of supporting, without failure, twice the weight of the employees, equipment and materials that may be imposed on the cover at any one time. Potential fall heights were measured to be greater than 18 feet above the concrete surface below. (b) On or about the 2-day period of April 30 - May 1, 2019, located at the south end of a commercial metal roof structure, one employee not tied off to a proper anchorage was exposed to fall hazards while removing screws from plastic skylight panels in that 10 uncovered plastic skylight panels were not capable of supporting, without failure, twice the weight of the employees, equipment and materials that may be imposed on the cover at any one time. Potential fall heights were measured to be greater than 18 feet above the concrete surface below.
