29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance: At the combo dumper area, employees were exposed to being caught-in, struck-by and amputation hazards where: a) Affected employees did not contact maintenance to perform lockout/tagout of the combo dumper cylinder pneumatic airlines. Instead, at least one affected employee disengaged the airline which exposed him to an amputation hazard and; b) The lockout/tagout procedure for the combo dumper was vague and did not clearly specify each procedural step for shut down and isolation of energy sources.
