29 CFR 1910.120(q)(9)(i): Members of an organized and designated HAZMAT team and hazardous materials specialist did not receive a baseline physical examination and/or were not provided with medical surveillance as required in 29 CFR 1910.120(f)(3)(i)(B): a) Ammonia refrigeration system: Employees designated as hazardous material technicians were not provided medical exams at least once every 12 months, on or about 4/10/19. b) Ammonia refrigeration system: Employees designated as hazardous material technicians were not provided a respiratory medical evaluation to determine their ability to wear an SCBA when responding to an ammonia release, on or about 4/10/19. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
