Text For Citation: 01 Item/Group: 004B Hazard:
29 CFR 1910.119(f)(1)(ii)(B): The employer's written operating procedures covering operating limits did not address the steps required to correct or avoid deviation: a) Ammonia refrigeration system: SOP 3.0 TCPA/Operating Procedures (USEPA, OSHA), for the ammonia refrigeration system did not list the complete steps to correct or avoid the consequences of deviation for low and high levels within intercooler IC-500. Subsequent actions to take after investigating the steps to correct the deviation were not listed, on or about 4/10/2019. b) Ammonia refrigeration system: SOP 3.0 TCPA/Operating Procedures (USEPA, OSHA), for the ammonia refrigeration system did not list the complete steps to correct or avoid the consequences of deviation of high discharge pressure or high suction pressure for the high stage compressors HSC-100 and HSC-1300. Subsequent actions to take after investigating the steps to correct the deviation were not listed, on or about 4/10/2019. c) Ammonia refrigeration system: SOP 3.0 TCPA/Operating Procedures (USEPA, OSHA), for the ammonia refrigeration system did not list the complete steps to correct or avoid the consequences of deviation of high pressure for low side suction on the low stage compressors LSC-200 and LSC-1400. Subsequent actions to take after investigating the steps to correct the deviation were not listed, on or about 4/10/2019. d) Ammonia refrigeration system: SOP 3.0 TCPA/Operating Procedures (USEPA, OSHA), for the ammonia refrigeration system did not list the complete steps to correct or avoid the consequences of deviation of low and high oil pressures and temperaturs for the high stage compressors HSC-100 and HSC-1300 and low state compressors LSC-200 and LSC-1400. Subsequent actions to take after investigating the steps to correct the deviation were not listed, on or about 4/10/2019. e) Ammonia refrigeration system: SOP 3.0 TCPA/Operating Procedures (USEPA, OSHA), for the ammonia refrigeration system did not list the complete steps to correct or avoid the consequences of deviation of low and high levels in the thermosyphon receiver TS-700. Subsequent actions to take after investigating the steps to correct the deviation were not listed, on or about 4/10/2019. f) Ammonia refrigeration system: SOP 3.0 TCPA/Operating Procedures (USEPA, OSHA), for the ammonia refrigeration system did not list the complete steps to correct or avoid the consequences of deviation of low and high levels in the pump package recirculator PPR-400. Subsequent actions to take after investigating the steps to correct the deviation were not listed, on or about 4/10/2019. g) Ammonia refrigeration system: SOP 3.0 TCPA/Operating Procedures (USEPA, OSHA), for the ammonia refrigeration system did not list the complete steps to correct or avoid the consequences of deviation of high discharge pressure or high suction pressure for the high stage compressors HSC-100 and HSC-1300. Subsequent actions to take after investigating the steps to correct the deviation were not listed, on or about 4/10/2019. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
