29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): A. Fabrication Department: On, or about, February 25, 2019, an employee suffered multiple partial finger amputations while attempting to cut copper plate on the Large Allsteel Shear. The barrier guard provided on the shear was not installed in accordance with guard opening guidelines in that the opening left between the guard and bed of the shear allowed access to both the hold-downs and shear blade. B. Fabrication Department: On, or about, March 7, 2019, the barrier guard provided on the Small Allsteel Shear was not installed in accordance with guard opening guidelines to prevent access to the hold-downs. C. Fabrication Department: On, or about, March 7, 2019, guards were not provided on the angle cutting station of the Austin Hastings iron worker, Model 70W-1019. Also, the plexiglass guard provided at the hole punch allowed access to the point of operation.
