29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. Location: Foiling room On or about January 3, 2019, lock-out tag-out devices were not used to lock out the electrical disconnect to the sloop foiling machine during cleaning and servicing activities thereby exposing employees to amputation hazards.
