29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards as required by paragraph (a)(1) of this section: The employer is failing to protect employees from fall hazards by failing to provide fall hazard recognition training associated with working on platforms supported by forklifts:. This was most recently documented at 300 Cloverly Rd., Fremont, Nebraska 68025 on or about January 15, 2019. The employer is failing to ensure that employees who were working on a supported scaffold that was over 10 feet above the ground which was attached to a JCB forklift were trained to the nature of fall hazards as a result of the condition of the scaffold that had a lack of fall protection.
