OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by or caught-in hazards: Location: Separator Room, North Roto-Screen Lockout procedures that had been developed and documented by the employer were not utilized by two employees consistently during their shift, exposing them to struck-by hazards and caught-in hazards. One feasible means of abatement for this hazard would include ensuring that employees utilize and comply with all elements of the lockout program.
