29 CFR 1910.269(u)(2): The employer didn't ensure that, when employees removed or insert draw-out-type circuit breakers, the breaker was in the open position. The employer did not also render the control circuit inoperable where the design of the equipment permitted. a) MCC ROOM: The employer did not ensure that the circuit would remain open and inoperable for the duration of the task when employees were inserting a 4160V breaker into switchgear, exposing employees to hazards from arc flashes including burns and heat inhalation. On December 5, 2018, three employee sustained burns resulting from an arc flash.
