29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a- Roof Level: Employees engaged in roofing activities were exposed to unprotected sky lights and fall hazards of approximately 22ft in which a means of sky light protection, such as personal fall arrest system, covers or a guardrail system was not utilized. Employees were not trained in the recognition and avoidance of fall hazards. Condition noted on or about 10/12/2018 b- Roof Level: Employees engaged in roofing activities were exposed to unprotected sky lights and fall hazards of approximately 22ft in which a means of sky light protection, such as personal fall arrest system, covers or a guardrail system was not utilized. Employees were not trained in the recognition and avoidance of fall hazards. Condition noted on or about 10/13/2018 c- Roof Level: Employees engaged in roofing activities were exposed to unprotected sky lights and fall hazards of approximately 22ft in which a means of sky light protection, such as personal fall arrest system, covers or a guardrail system was not utilized. Employees were not trained in the recognition and avoidance of fall hazards. On 10/14/2018, an employee fell through an unprotected sky light and suffered fatal injuries as a result of the fall.
