29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) On or about October 25, 2018, at the above located workplace, the employer did not provide effective training to each authorized employee who performed cleaning on the Rheon, V4 Free Divider with Cross Roller, Model VX122 machine. A sanitation employee did not lockout the machine but used magnets to bypass interlocks to operate the machine during cleaning. The employee was thereby exposed to hazards associated with moving parts and the unexpected start-up of the machine.
