29 CFR 1910.146(k)(1)(v): 29 CFR 1910.146(k)(1)(v): The employer who had designated rescue and emergency services in their permit required confined space entry program did not provide the selected rescue team or service with information and access to all permit spaces from which rescue could be necessary, so that the rescue service could develop a rescue plan and be familiarized with the hazards, tasks and required equipment for each permit required space identified if summoned to perform rescue operations. Boiler Unit #2, 331 Southwest Cutoff Millbury, MA: On or about October 14, 2018 and at times prior, the employer did not provide the rescue team with information and access to the boiler units in order to evaluate the hazards, tasks and required equipment need to develop a rescue plan and to perform a rescue. Two employees were trapped inside the boiler for approximately four hours after the suspended scaffold (Skybridge) malfunctioned.
